Genesis Medical Health is partnering with and alongside Rockland Urgent Care to offer a Primary Care Practice. We are pleased to announce the newest member to our team Dr. Butler who will be the Primary Care Practitioner (PCP). Dr. Butler, formally of HealthCheck, looks forward to providing you with continued personalized, comprehensive health care focusing on wellness and prevention. In addition to all you have been accustomed to, we will now have direct access to state-of-the-art diagnostic equipment and extensive services on the premises, such as:

• Stitches and Suturing Capabilities

• DOT Drug Testing

• FMSCA DOT Driver Medical Exams (CDL Physicals)

• NYS DMV Vision Screening

• NYS Medical Marijuana Program

• Dedicated Exam, Trauma, and Treatment Rooms

• Minor fractures and dislocations

Our schedulers are available every day and will do their best to accommodate you. Booking an appointment is essential to ensuring all patients receive the time they require for quality medical care.  

Before your visit, please notify your health insurance company of your new primary care provider, if required. We also request that you contact your previous physician and specialists and request that a copy of your medical record be sent to us.

Please download our enclosed forms here           and fill out and bring them with you to your appointment. 

Please bring your health insurance identification card as well as a photo I.D.     

Please bring a complete list of all of your medications, as well as the strength and dose of each one.
